Case Managers, Nurses, Physicians
Some highlights of the service we extend includes:
- Scheduled appointment offered within 24 hours of referral
- Home exercise program will be initiated at first appointment and modified as needed
- Patient will be discharged if not progressing or if patient plateaus
- Estimated length of treatment included in initial evaluation
- Progress notes monthly or when patient returns to M.D. (may request more frequently)
- Pre-Op and Post-Op P.T.
- Pre-schedule P.T. for treatment after outpatient procedures
- We will schedule follow-up visits and notify physician's office of absences / cancellation / rescheduling of patient
- Assess vocational needs to determine what modifications may be necessary to enable employee to return to work
- Willing to take video to job site to modify job site for return to work
- Will work with case managers to find a way to get employee back to work at least part-time (not feasible in some industrial situations)
- We can provide in-services for many topics that may benefit Nursing Staff
