Eastern Shore Physical Therapy participates in most health care plans. As a courtesy to patients, Eastern Shore Physical Therapy will file for all insurance claims.
Patients are responsible for all co-pays and deductibles. We ask that these payments be made on a weekly basis. Cash, Personal Checks, Debit Cards, MasterCard, Visa and Discover are acceptable methods of payment.
To avoid decreased insurance benefits, patients are advised to check with their insurance company prior to services being rendered. If an insurance plan requires authorization or referral, it is the patient's responsibility to ensure that this is completed.
Third party patients or patients who have been involved in a motor accident and carry private health insurance are required to file for their health insurance.
